Worked example: from 2,400 meters to 42.4
Say you run the Cooper test and cover 2,400 meters in 12 minutes. The formula is VO2 max = (distance - 504.9) / 44.73, where distance is in meters. Subtract first: 2400 - 504.9 = 1895.1. Then divide: 1895.1 / 44.73 = 42.4 ml/kg/min (42.37 unrounded).
That puts a 2,400 m runner just inside the 'average' band in the calculator, which draws lines at 32, 42, 52 and 63. The thresholds are unforgiving: 50 meters more distance (2,450 m) gives 43.5, while 50 meters less (2,350 m) gives 41.2, which drops you below average. Every 44.73 meters you cover adds roughly 1 ml/kg/min, so about 9 meters per minute of running pace is the exchange rate.
For the heart-rate method, the same arithmetic habit applies. A person with a max heart rate of 180 and a resting rate of 60 gets VO2 max = 15.3 x (180/60) = 15.3 x 3.0 = 45.9. Lowering resting heart rate is the only lever you control: at the same max of 180, a resting rate of 50 gives 15.3 x 3.6 = 55.1.
Where both estimates break down
The Cooper formula was built on Air Force personnel aged roughly 20 to 52 in 1968, so treat its output as a band, not a measurement. Two failure modes matter. Treadmill or track pacing errors shift the result hard: running 2,400 m with a 400 m tailwind-assisted split versus a genuine solo effort can easily swing the answer by 3 to 5 ml/kg/min. And because the formula assumes a roughly steady 12-minute effort, a runner who goes out too fast and fades gets punished twice, once in distance and again in pacing accuracy.
The Uth heart-rate method (VO2 max = 15.3 x max HR / resting HR) never touches distance at all, so it cannot see the difference between a trained runner and an untrained one with identical heart rates. It also inherits two noisy inputs: wrist-based resting heart rate readings can read 3 to 8 beats low overnight, and max HR estimates from the 220-minus-age rule are wrong by more than 10 beats for many people. A max HR of 175 versus a true 190, at the same resting rate of 60, gives 44.6 versus 48.5.
Practical rule: use the run result when you can pace a flat 12 minutes accurately, use the heart-rate estimate as a cheap weekly trend, and treat a 5 point gap between them as normal rather than a bug.
